Description:

Flagler Cares offers multiple community programs to Flagler residents to fill gaps in the community's safety net. We offer:

Therapy Services. We offer therapy services tailored to meet individuals where they are in their journey. We specialize in substance use treatment, trauma-focused therapy, and support for anxiety, depression, and other mental health concerns for individuals, couples, and families.

Partnered for Success Program. Through the Partnered for Success Program, staff help individuals and families build a path toward economic stability and long-term success. Participants partner with a coordinator to set goals, develop a plan, overcome barriers, and connect to services that support financial and health stability.

Coordinated Opioid Recovery (CORe). The Coordinated Opioid Recovery (CORe) initiative brings together multiple agencies working collaboratively to address the many needs individuals may face throughout recovery. While the primary focus is supporting those with opioid use disorder, services are also available for individuals experiencing other substance use disorders.

Disability Program (SSI/SSDI Benefits). Flagler Cares staff can screen individuals for disability claim eligibility and either provide advice on the process or directly assist individuals through the process if they qualify for our SSI/SSDI Outreach, Access, and Recovery (SOAR) program.

Prevention & Youth Opportunities. Flagler Cares provides youth activities that build advocacy, healthy coping,
leadership, and resilience skills while providing connections and a safe space to grow. Through evidence-based programs and trained staff, we use a risk reduction and protective factor model for substance use prevention, effectively reaching both youth and adults.

Behavioral Health Navigation. Flagler Cares staff will help individuals understand their health insurance benefits and identify and connect them with appropriate behavioral health providers to meet their needs and help them overcome barriers to accessing appropriate care.

Information & Referral. Flagler Cares staff can conduct a phone interview to gain an understanding of individuals!€™ needs and refer them to specific programs and services, either at Flagler Cares or available through other community organizations.

All services are assessable to all. Each program has specific requirements that include: insurance, sliding fee scale, grant funded and no cost services.
