Proof of residency: driver's license or current utility/cable/phone bill.
Proof of dependent eligibility: Birth certificate for each child or proof of legal guardianship.
Proof of child(ren)'s date of birth: Birth certificate for each child.
Proof of currently receiving Medicaid, food assistance (SNAP or WIC), cash assistance (TANF), or Free and Reduced-Price School Meals.
